Background of the Study:
Rural healthcare accessibility is a critical determinant of overall public health, particularly in regions where infrastructural challenges and poverty are prevalent. In Borno State, rural areas face significant barriers to accessing healthcare, including long distances to facilities, poor transportation networks, and limited availability of medical services. These challenges contribute to delays in seeking care and poorer health outcomes among rural populations (Okeke, 2023). Rural residents are often forced to travel great distances at high costs to reach the nearest healthcare center, which can deter timely treatment and exacerbate health conditions. Furthermore, the quality of care in rural areas is frequently compromised by a shortage of healthcare workers, outdated equipment, and inadequate supplies (Ibrahim, 2024).
The relationship between healthcare accessibility and health outcomes is well documented; when access is limited, disease incidence, morbidity, and mortality tend to be higher. In Borno State, the challenges of rural healthcare accessibility are compounded by ongoing economic hardships and infrastructural deficits resulting from prolonged conflict and neglect. These factors create an environment where preventable and treatable conditions go unmanaged, leading to a higher burden of disease and lower life expectancy (Aminu, 2024). Recent research underscores the need for comprehensive strategies that improve both the availability and quality of rural healthcare services, which in turn can enhance health outcomes and reduce disparities between rural and urban populations (Fatima, 2025). This study aims to evaluate rural healthcare accessibility in Borno State and its impact on health outcomes, with a view to recommending integrated interventions that can bridge the urban–rural healthcare gap.
Statement of the Problem:
In Borno State, rural populations continue to experience significant challenges in accessing healthcare, resulting in poorer health outcomes compared to their urban counterparts. Factors such as inadequate transportation infrastructure, long distances to healthcare facilities, and limited availability of skilled healthcare professionals contribute to delayed or foregone medical care (Umar, 2023). These access barriers are further compounded by economic hardship and under-resourced health facilities, which result in low service utilization and high morbidity and mortality rates. The existing disparities in healthcare accessibility between rural and urban areas highlight the urgent need for targeted interventions to improve rural healthcare services. Without adequate access, rural residents are at a higher risk for preventable diseases, leading to a continued cycle of poor health and economic disadvantage (Saleh, 2024). This study seeks to evaluate the extent of rural healthcare accessibility in Borno State and to analyze its impact on key health outcomes, thereby providing evidence-based recommendations to address these critical gaps.
