Background of the Study:
Maternal healthcare access is a critical determinant of maternal and neonatal outcomes. In Zamfara State, high maternal mortality rates persist, largely due to limited access to quality maternal healthcare services. Factors such as poor infrastructure, inadequate healthcare facilities, and low levels of education contribute to the challenges faced by pregnant women in obtaining timely and appropriate care (Chukwu, 2023). Rural communities in Zamfara State are particularly vulnerable, where long distances to healthcare facilities, high transportation costs, and cultural practices often delay or prevent women from seeking care during pregnancy and childbirth. Additionally, economic constraints exacerbate these issues, as many families cannot afford the costs associated with antenatal care, skilled birth attendance, or emergency obstetric services (Ibrahim, 2024).

Government initiatives aimed at improving maternal health have included the establishment of primary health centers, free maternal healthcare programs, and community-based interventions. However, the effectiveness of these programs is hindered by challenges in implementation, limited resources, and inadequate coordination among healthcare providers. The overall quality of maternal healthcare in Zamfara State is further compromised by the shortage of skilled healthcare workers and a lack of essential medical supplies. These barriers collectively contribute to poor maternal and neonatal outcomes, including high rates of maternal mortality, low birth weight, and neonatal deaths (Fatima, 2025). Understanding the factors that affect maternal healthcare access and outcomes is essential for designing targeted interventions that can improve health indicators and reduce preventable maternal deaths in Zamfara State.

Statement of the Problem:
In Zamfara State, maternal healthcare access is severely limited, contributing to poor health outcomes among mothers and newborns. Despite government initiatives to provide free or subsidized maternal care, many women still face significant barriers due to geographical distance, economic constraints, and cultural practices that delay healthcare-seeking behavior (Umar, 2023). The lack of skilled birth attendants and inadequately equipped health facilities further exacerbate the risks associated with childbirth. As a result, maternal mortality and morbidity rates remain unacceptably high, with adverse outcomes that affect not only the mothers but also the long-term health of their children. The challenges are particularly acute in rural areas, where infrastructural deficits and low health literacy impede access to essential maternal health services (Saleh, 2024). This situation underscores the need for a comprehensive evaluation of maternal healthcare access and outcomes in Zamfara State, with the goal of identifying effective strategies to reduce maternal and neonatal mortality.

  • To assess the level of access to maternal healthcare services in Zamfara State.
  • To evaluate maternal health outcomes, including maternal mortality and neonatal health indicators.
  • To recommend interventions to improve maternal healthcare access and outcomes.

Research Questions:

  • What are the primary barriers to accessing maternal healthcare in Zamfara State?
  • How do these barriers affect maternal and neonatal health outcomes?
  • What strategies can enhance the quality and accessibility of maternal healthcare?

Research Hypotheses:

  • H1: Limited access to healthcare facilities is significantly associated with higher maternal mortality.
  • H2: Economic constraints and transportation issues negatively impact maternal healthcare utilization.
  • H3: Strengthening healthcare infrastructure will improve maternal and neonatal outcomes.

Scope and Limitations of the Study:
This study focuses on maternal healthcare access and outcomes in selected rural and urban areas of Zamfara State. Limitations include potential data gaps in maternal health records and the difficulty in isolating specific barriers from the broader healthcare system challenges.

  • Maternal Healthcare: Medical services provided to women during pregnancy, childbirth, and the postpartum period.
  • Maternal Mortality: The death of a woman during pregnancy or within 42 days of childbirth.
  • Neonatal Outcomes: Health indicators related to the well-being of newborns.
