Background of the Study
Community health workers (CHWs) are essential in bridging the gap between formal healthcare systems and underserved communities. In Kano State, CHWs play a critical role in promoting immunization by educating caregivers, facilitating vaccine administration, and conducting follow-up visits. Their close relationship with local communities and cultural competence enables themto address barriers to immunization, such as vaccine hesitancy, misinformation, and logistical challenges (Ibrahim, 2024; Adeyemi, 2023).

CHWs are often the first point of contact for health-related issues, and their efforts are crucial in increasing immunization coverage among children. They conduct door-to-door visits, organize community meetings, and collaborate with local leaders to raise awareness about the importance of vaccines. Research has shown that areas with robust CHW programs tend to have higher immunization rates and better overall health outcomes. However, challenges such as insufficient training, inadequate resources, and high workloads may limit the effectiveness of CHWs in Kano State (Chukwu, 2024).

This study aims to evaluate the role of community health workers in improving immunization rates in Kano State by examining their practices, challenges, and impact on vaccine uptake. Through a mixed-methods approach that includes quantitative analysis of immunization records and qualitative interviews with CHWs and community members, the research will provide a comprehensive understanding of how CHWs influence immunization coverage. The findings are expected to inform strategies to strengthen CHW programs, enhance training and support mechanisms, and ultimately improve immunization rates in Kano State.

Statement of the Problem
In Kano State, immunization coverage remains suboptimal in many communities despite efforts to promote vaccine uptake. One critical factor contributing to this challenge is the limited effectiveness of community health workers (CHWs), who are tasked with delivering immunization services and education at the grassroots level. CHWs often face challenges such as inadequate training, insufficient supplies, and high workloads, which hamper their ability to effectively reach and educate all segments of the population (Lawal, 2023).

The lack of standardized protocols and continuous professional development for CHWs further exacerbates the problem. As a result, vaccine hesitancy persists in some communities, and many children miss scheduled immunizations, increasing the risk of vaccine-preventable diseases. Additionally, gaps in coordination between CHWs and formal healthcare facilities may lead to inconsistencies in service delivery. These issues collectively undermine the success of immunization programs in Kano State.

This study seeks to address these challenges by evaluating the role and performance of CHWs in improving immunization rates. By identifying the barriers that CHWs face and assessing their impact on immunization coverage, the research aims to provide actionable recommendations for enhancing CHW effectiveness and strengthening immunization programs in the region.

Objectives of the Study

Research Hypotheses

Scope and Limitations of the Study
The study focuses on CHWs operating in urban and rural areas of Kano State. Data will be collected through surveys, interviews, and review of immunization records. Limitations include potential self-report bias and variability in CHW program implementation.
