Background of the Study
Heart failure (HF) is a growing health concern worldwide, with high rates of hospitalization and readmission, contributing significantly to the global healthcare burden. In Nigeria, the situation is no different, as heart failure remains one of the leading causes of morbidity and mortality, with frequent hospital readmissions due to complications and inadequate management (Eze et al., 2023). National Hospital in Abuja, as a key healthcare provider in the country’s capital, treats a large number of patients with heart failure, many of whom experience multiple readmissions within short periods. These readmissions are often the result of poor disease management, insufficient patient education, and lack of follow-up care after discharge (Akinola et al., 2024).
Nurses play a crucial role in reducing hospital readmissions for heart failure by providing timely interventions, including patient education, medication management, lifestyle counseling, and post-discharge follow-up (Adedeji et al., 2023). Nurses’ interventions are designed to enhance patient understanding of their condition, encourage adherence to treatment regimens, and promote lifestyle modifications that can prevent exacerbations. The relationship between nurse-led interventions and reduced hospital readmissions has been highlighted in various studies, suggesting that improved care coordination and continuous support can significantly decrease the likelihood of patients being readmitted to the hospital (Olutunde & Chukwu, 2023).
However, despite the recognized importance of nursing interventions, there has been limited research examining their specific impact on hospital readmissions for heart failure in the Nigerian context, particularly at National Hospital, Abuja. It is essential to explore how nursing interventions can be optimized to reduce readmission rates and improve the quality of care for heart failure patients in this setting.
Statement of the Problem
Heart failure is a major health issue in National Hospital, Abuja, and frequently leads to hospital readmissions due to poorly managed symptoms and lack of continuity of care. While nurses are integral to the management of heart failure, there is limited research on the specific impact of nursing interventions in reducing hospital readmissions for these patients. The lack of systematic studies on the effectiveness of these interventions in Nigeria limits the development of targeted strategies that could improve patient outcomes and reduce the strain on healthcare resources. Therefore, it is critical to assess how nursing interventions can help mitigate hospital readmissions and improve the overall management of heart failure in National Hospital, Abuja.

Scope and Limitations of the Study This study will focus on heart failure patients admitted to National Hospital, Abuja, and the nursing interventions aimed at reducing hospital readmissions. The research will explore the effectiveness of different interventions and the challenges nurses face in implementing them. However, the study will be limited to a specific hospital, and findings may not be generalizable to other healthcare institutions in Nigeria. Additionally, the study may face limitations such as participant bias, limited sample size, and data access constraints.
